Job Description- Assists the physician in evaluating clients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests

- Treats clients to relieve pain and/or develop or restore function to maintain maximum performance

- Makes arrangements for out-client services, which cannot be given in the home

- Records and reports to the physician the client’s reaction to treatment and/or any changes in the client’s condition

- Instructs clients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.)

- Instructs the family in the client’s total physical therapy program

- Attends physical therapy staff meetings and in-service presentations

- Attends case conferences and in-service programs as indicated

- Participates in staff and Agency development activities

- Prepares an assessment of the client’s condition and periodic evaluations and summaries, develops a plan of care and problem list, and writes clinical/progress notes on the date of the visit and incorporates same in the client’s record no less than weekly

- Participates in the development and periodic revision of the physician’s plan of treatment

- Communicates effectively with all those providing care

- Confirms, on a weekly basis, the scheduling of visits with the DON to coordinate necessary visits with other personnel

- Notifies Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings which will affect agreed services with the Agency

- Participates with staff, client, and physician in discharge planning activities and completes the physical therapy discharge summary within the designated framework of Agency policies

- Reviews and evaluates physician's or podiatrist's referral and patient's medical record to determine physical therapy required.

- Plans and prepares a written treatment program based on the evaluation of available patient data.

- Performs patient tests, measurements, and evaluations, such as range-of-motion and manual muscle tests, gait and functional analyses, and body parts measurements, and record and evaluate findings to aid in establishing or revising specifics of treatment programs.

- Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury or loss of body part.

- Administers manual therapeutic exercises to improve or maintain muscle function, applying precise amounts of manual force and guiding patient's body parts through selective patterns and degrees of movement. Instruct, motivate and assist patient in non-manual exercises, such as active regimens, isometric and progressive resistive, and in functional activities using available equipment and assisting and supportive devices, such as crutches, walkers, canes, orthoses and prostheses. Administer treatment involving application of physical agents, such as heat, light, cold, water and electricity. Administer traction and massage. Evaluate, fit and adjust prosthetic and orthotic devices and recommend modifications to the orthotist/prosthetist. - Observes, records and reports to the physician or podiatrist the patient's treatment, response and progress.

- Instructs other health team personnel including, when appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient.

- Instruct patient and family in total physical therapy program.

- Prepares clinical and progress notes for the clinical record.

- Supervision of the physical therapist assistant as follows:

A) Must be accessible by telephone to the physical therapist assistant at all times while the physical therapist assistant is treating patients.

B) Must provide on-site supervision every six visits. The supervisory visits may be made either when the physical therapist assistant is present so that the supervisor may observe and assist, or when the physical therapist assistant is absent so that the supervisor may assess relationships and determine whether goals are being met.

C) Supervision does not constitute treatment.

D) The supervisory visit should include a complete on-site functional assessment, an on-site review of activities with appropriate revision of treatment plan, and an assessment of the utilization of outside resources.

Job Requirements

Qualifications:

Graduate of a physical therapy school approved by:
- The American Physical Therapy Association; or
- The Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association; or
- The Council on Medical Education of Hospitals for the American Medical Association
- Licensed or registered by the state
- Preference of at least one (1) year of institutional experience

Special Requirements:

(Preferred) - Must have a car with required insurance coverage and a State driver’s license

Functional Abilities:

- Must be able to read 12 point or larger type
- Must be able to hear and speak in a manner understood by most people
- Must be able to stoop and bend
- Must be able to lift and carry effectively so as to be able to perform the above-listed functions
- Must be able to travel to prospective clients’ residences
